NY Connects is a collaborative effort by the New York State Office for the Aging (NYSOFA) and New York State Department of Social Services (NYSDSS) to provide access to information and assistance about long term care. Through the state other counties including Putnam County are establishing consumer- centered resource points for information about long term care for all individuals in need of services, their caregivers and health professionals.
