Mission Statement
Putnam County Office for Senior Resources is responsible for stimulating, promoting, coordinating and administering local programs and services for older Putnam County residents. In carrying out this responsibility, the Office for Senior Resources’ primary emphasis is on the development of long-term care programs and services necessary to meet the long term care needs of Putnam County’s elderly and to support informal caregivers.

Services Offered
The Older Americans Act (OAA) and New York State regulations require that local AAAs develop and submit an Area Plan to the New York State Office for the Aging (NYSOFA).
The Putnam County Office for Senior Resources operates 4 senior centers which serve hot nutritious noontime meals, offer companionship, wellness and exercise classes, arts and crafts, games, fall prevention programs, nutrition education, nutrition and health counseling, blood pressure readings, and talks by local physicians and pharmacists, etc. Transportation and shopping assistance is offered for those who have no other transportation.

This federally funded program is designed to help with the payment of heating/utility bills. The Office for Senior Resources certifies applications for residents 60 years of age and older.

An attorney is available to provide legal consultation to include wills, health care proxies, and powers of attorney. An appointment is required.

The Outreach Program assists the isolated and frail elderly in the community by obtaining services and providing them with information and referral as necessary. Outreach reaches OUT into the community to the unserved and underserved seniors of Putnam County, by assisting in certifying or re-certifying eligible seniors for programs such as Medicaid, Food Stamps, and/or the Medicare Savings Program. Exercise, crafts, and games are important social activities that help nurture us physically, mentally, and spiritually. These programs come in many forms.
Arts and crafts activities such painting and ceramics help generate the creative juices.
Games and other social activities include billiards, bingo, various card games, puzzles, mahjong, Wii, book clubs, brain fitness, coffee and conversation, movies.
We offer evidence based fall prevention programs such as A Matter of Balance, Tai Chi for Arthritis and Bingocize. Other exercise programs such as Chair Zumba, Water Bottle Workout and Ballroom Dancing all serve to improve our balance, mobility and get that heart rate going in the right direction.
Some programs are offered on line for those who are homebound or unable to come in on a particular day.

Provides limited medical transportation for getting to and from doctor and hospital appointments for seniors who have no other means of transportation and who otherwise could not obtain needed medical care. Transportation is provided within Putnam County and is also available to certain destinations outside of Putnam County.
Transportation is provided by senior volunteer drivers from Putnam SeniorCorps.

The Putnam County Office for Senior Resources has a Health Insurance Information Counseling & Assistance Program (HIICAP) Coordinator to help you sort through the maze of insurance options to find the correct answers for you! HIICAP provides free, accurate and unbiased information, counseling, assistance and advocacy on Medicare, private health insurance, and related health coverage plans. HIICAP helps Medicare beneficiaries, their representatives or persons soon to be eligible for Medicare.

A home care program providing a variety of non-medical services to the elderly with functional impairments and who require assistance with bathing, eating, toileting, dressing, light housekeeping, etc.
This is a cost-sharing program based on income. Need and eligibility is determined after an assessment by a case manager.

Consumer directed care gives you more choices in meeting your care needs and allows you to be in control of your home care service. The consumer or designated person is responsible for the selection, hiring, supervision and, if needed, termination of their aides.
With consumer directed care, you can choose who to hire for your care, including family (not including spouses) and friends. You have a large role in shaping your plan of care to help you remain safely in your home and community.

EPIC was developed to assist eligible NY State residents 65 or older with the high cost of prescription drugs. Seniors who meet income guidelines and who are not already covered by Medicaid or other programs which supplement the cost of prescription drugs may be eligible.

Emphasizing wellness at all four friendship centers monthly by utilizing community resources such as volunteers to do blood pressure readings, local physicians to speak on health topics, Putnam County Health Dept. to give flu and pneumonia shots, and local pharmacists to answer questions concerning medications. “Please come and participate…….remember, HEALTH IS WEALTH!!”

Falls are the leading cause of both fatal and nonfatal injuries among older adults.
OSR offers a number of programs to promote awareness of the risks related to falls and to provide tools to seniors to offset these risks. These include evidence based fall prevention programs such as A Matter of Balance, Tai Chi for Arthritis and Bingocize which have been developed at universities and have demonstrated that participation can reduce fall risk. OSR uses the Steadi Tool Kit to assist seniors in the measurement of their own individual fall risk. Each Fall, OSR participates in the National Falls Prevention Week with gatherings at varying locations to promote awareness on Fall Awareness and to demonstrate useful techniques and programs for seniors to access.

Putnam NY Connects serves older individuals and individuals with disabilities of all ages, and their caregivers, with a link to community long term services and supports to help them remain in their home as long as possible.

The Adult Day Services program is designed to meet the needs of functionally impaired adults with cognitive and/or mobility issues through individual plans of care. This program provides a variety of health, social and support services in a protective setting during daytime hours. Keeping our participants actively engaged in the community provides their caregivers with peace of mind and much needed and deserved time for themselves.

Nutrition is a key component of the Social Determinants of Health services provided by the Office for Senior Resources. It is also a key element of our mission to serve the seniors of Putnam County. The nutrition services we provide include:
Senior Center Lunch: hot nutritious noontime meals as part of our congregate meal program at our 4 Friendship Centers on Monday through Friday, excluding holidays and days we are closed due to training, weather or other emergencies.
Home Delivered Meals: a hot, nutritious meal is delivered directly to the homes of sick or homebound elderly. A physician’s referral and home assessment is required. This program can also include the delivery of frozen meals to the homes of eligible seniors. Meals are provided for up to 7 days per week depending on individual circumstances. An assessment for frozen meals is also required.
Some special diets can be incorporated into meals served.

A certified aide assists seniors who are unsteady in the shower, having trouble navigating the bathtub, or you are a caregiver struggling in helping your loved one with personal care needs. SOAP offers walk-in shower and laundry facilities at our senior centers in Mahopac and Putnam Valley. Need and eligibility is determined after an assessment by a case manager.

The Putnam Office for Seniors Resources assists caregivers in their effort to maintain the quality of life for their loved ones. Program services include counseling, information and referral, support groups, educational workshops, and the Caregiver Resource Center. The Center lends books and pamphlets and provides links to other resources to assist in the caregiving journey.

Putnam SeniorCorps invites adults 60 and older to use their life experiences and skills to volunteer and assist others close to home in a wide variety of community organizations. Volunteers are matched with assignments of their own choosing. The Program provides orientation, training, and transportation. Volunteers lead more active, healthy, dynamic lives, full of the excitement of participation while serving your community.

Older Americans Act (OAA)

The Older Americans Act (OAA) and New York State regulations require that local AAAs develop and submit an Area Plan to the New York State Office for the Aging (NYSOFA). Each AAA receives resources through state and federal funds to fulfill its role for planning and coordination in its planning and service area (PSA). The Putnam County Office for Senior Resources is responsible for developing and administering a four-year Plan for Services for 2024-2028 that provides goals and objectives related to assisting older residents, their families, and caregivers.

*NEW* 2025 Special Enrollment Period
People with Extra Help, MSP (Medicare Savings Program), or Medicaid can no longer switch from a Medicare Advantage Plan to another Medicare Advantage Plan unless it’s a D-SNP (Dual-eligible Special Needs Plan).
*Starting January 1, 2025 * Once per month, people with Extra Help, MSP, or Medicaid can enroll into a stand-alone Prescription Drug Plan or integrated D-SNP.

January 1—March 31 is the Medicare Advantage Open Enrollment Period.
During the Medicare Advantage Open Enrollment Period, Medicare Advantage enrollees can switch to Original Medicare or to a different Medicare Advantage Plan from the one they are currently enrolled in. You can make one change with new coverage effective first of next month.
CAREGIVERS
Do you provide unpaid care to a family member, friend, or neighbor who has an illness, disability, memory loss, injury, or special need? If you answered yes to any of these questions, you are a caregiver! Are you feeling overwhelmed, constantly worried, tired, sad, easily irritated, and/or extremely stressed? You are not alone and there is support available. If you would like to learn more about resources, tools, and supports available for caregivers, please answer the following questions to get connected to the right starting place.
- Are you over the age of 18 and caring for an older adult (age 60+)?
- Are you over the age of 18 caring for an individual (any age) with Alzheimer’s disease or a related disorder (e.g., dementia, traumatic brain injury, mild cognitive impairment, chronic traumatic encephalopathy)?
- Are you over the age of 55 and living with and caring for a child (not biological) under the age of 18?
- Are you over the age of 55 and living with and caring for an adult (can be a biological child) aged 18-59 with a disability?
If you answered yes to any of the questions above, you are eligible for the National Family Caregiver Support Program (NFCSP). If you would like to learn more about how this program can assist you, please click this link for a short screen for an initial assessment of your wellbeing, and a Program Specialist will reach out to discuss the results. The evidence-based screener asks you to reflect on your experiences and respond to a series of statements based on identified rating scales in each section. This screener is used to help you and the Program Specialist better understand your situation and define an individualized plan to meet your unique caregiving needs.
If you answered no to all the questions above, or you are not interested in taking the evidence-based screen, there is still help available to you through the Any Care Counts - New York (ACC-NY) Campaign which recognizes and supports the millions of unpaid caregivers across the state! Through ACC-NY you can discover your caregiver intensity score by taking the ARCHANGELS Caregiver Intensity Index (CII). It takes just 2 minutes to complete. You will find out whether you are “in the red," "yellow" or "green," and be connected to trusted resources.

Medicare - Open enrollment period: October 15, 2023 through December 7, 2023
New for 2023 - Social Security Enrolls You In Original Medicare (Part A and Part B) Medicare is health insurance for people age 65 or older. Certain people younger than age 65 can qualify for Medicare, including those with disabilities and those who have permanent kidney failure.
If you’re eligible at age 65, your “Initial Enrollment Period” begins three months before your 65th birthday, includes the month you turn age 65, and ends three months after that birthday.
